Health Department Wants Your Input

In order to measure community awareness about services, the health department is using the month of October to collect responses at www.surveymonkey.com/s/PublicHealthAwareness. 

The survey will run until Oct. 31 and should take less than five minutes to complete. 

At the end of the survey, participants will have the option to include contact information to be eligible to win one of two $50 Target gift cards. 

Winners will be required to visit the health department to pick up the gift card. 

Individual responses will be kept confidential and will be used solely for internal purposes.
